I think the song is about a guy who has gone down the wrong road. Although he sees that the place he is headed (the afterlife, heaven) is perfect, he is not ready to be there so he wants to change and have a chance to be the person he wants to be. "Loved ones back home all crying 'cause they're already missing me,
I pray by the grace of God that there's somebody listening" probably means that the road he is on is ulimately hurting the ones he cares about and that he hopes that people will take the message to heart and make a change in their lives too.
