My take on it is that his girl is on her deathbed after a long illness…. He says you don't need to medicate, you're strong without it…and basically they both know she's going to die and she's the stronger of the two because he's trying to comfort her telling her not to be afraid, but really she's the one that's been helping him through it all…he's asking her to hold on because he knows he's not as strong without here there…then she dies and leaves him alone…he says "it's not a better place" referencing how people always say "they're in a better place now" after someone dies…
