For me, this song is about when you get to that point in your life where you have been through so much that your mind is telling you're hopeless and worthless, and you feel horrible, trapped, like you are falling down, and like you can't get up or move on. I also see that this song could be about being on drugs and how you feel when you are addicted. However, that is my opinion, and many people will see this song differently, or they may see it in more than one way like I do.
