To me this song is about an unrequited love. For whatever reason they were never together and now all she does is think about him and worry about him. Her feelings for him have never changed but for whatever reasoning they are still not together rather it be a complication of life, or of expression. Anyone who has truly loved knows how awkward it can be to tell the other that you infact do love them.
