"I try to hold on to this world with everything I have, But I feel the weight of what it brings" this is saying that he/she tries to stay alive on this world, but in the end it just brings more pain, so he feels the "weight of what it brings".
"The many trials that seem to never end." is referring to the trials of life or the harshness of sins repeating tragedies. "His word declares the truth." Obviously, that through everything you go through in life the Word of God is the only thing you can fully believe in. "Troubled soul don't lose your heart, Cause joy and peace he brings, And the beauty that's in store, Outweighs the hurt of life's sting." this is saying that no matter what happens to you don't loose faith in God for the events of the future will be overcome the hurt of sin.

What this song means to me. In the bible it says there will be no more pain and no more sorrow no more suffering, when we go to heaven. Jesus will take that all away. For jeremy he wrote this for his first wife who died of overan cancer and he was praying to god saying why did you take her away from me. She was really sick telling jeremy that she was ready to meet jesus face to face. So be ready cause he's coming back.
